The villager is unlikely to turn to the doctor for advice on how to build a latrine and the doctor is unlikely to be listened to if he gives that advice. Similarly, advice on nutrition and hygiene will often fall on deaf ears. The doctor will be listened to politely and respectfully but his advice will be ignored since it is perceived as being significantly outside his area of recognized wisdom. The competence of the doctor overlaps and to some extent grows from that of the traditional healer. Despite the problems, possibilities do exist for establishing community health workers and, as in the case of the UNICEF project, for reinforcing and developing the traditional competences of local healers.
In the Hunza valley in northern Pakistan, there already exists the practice of collecting human faeces in simple latrines for use on the fields at sowing time. They have discovered that in the dry air and harsh sunlight of the mountains human and animal dung soon dries out and ‘burns’, and so loses much of its fertilizing potential. Soil fertility is a crucial issue for the Hunza people since arable land is scarce and the soil cover often thin. In neighbouring Baltistan and Ladakh, which are at even higher altitudes, the indigenous latrines are more sophisticated.
