These and other research-based reports, plus strong advocacy by parents and professionals, led to increasing efforts to provide early identification of limited hearing and prompt, family-focused early intervention—nationally and internationally. Of course, the challenges faced differ by location, and the way in which early intervention is provided will necessarily vary depending on cultural expectations and traditions. Delivery of early intervention services is challenged in industrialized as well as still-industrializing countries by a general lack of trained specialists with experience related to deaf and hard-of-hearing children, especially during the early years of life.
1 For example, little Gregory has typical hearing, but Carlos has significantly limited hearing and was probably responding to vibrations caused by the door slamming. Because it is difficult to identify a baby’s hearing status just by observing his or her behavior, in the past many children were 2 or 3 years old or even older before limited hearing abilities were discovered. In fact, one of us (back in the late 1970s) documented the language growth of 3-year-olds who were just beginning intervention programming (Spencer Day, 1986).
These assessments use more detailed, in-depth methods and are more sensitive versions of the general kinds of tests used in screening. For example, the AEP approach can be used in complex and detailed ways to assess responses from different places in the hearing system (including the inner ear, the hearing nerve, and various parts of the brain involved in hearing) and using a variety of types of sounds. These extensive tests will give a more complete and accurate picture of what the baby can or cannot hear.
