The patient’s body is rotated to lie more prone than supine (pillows or other supports may be employed).
The patient’s stated name is compared to the name and patient number on the armband, the chart, and the addressograph plate. The patient is asked the name of the surgeon, the type of surgical procedure that he or she anticipates having, the location (site on the body), and the laterality (side) where the procedure will be performed. The location is identified by the patient and corroborated by the perioperative nurse and/or the surgeon. The site is marked with indelible ink. Even when the procedure will be performed on both sides of the body (either left or right but not both sides on the same day), the site for the procedure is marked (boldly), the designated site/side stated on the consent for surgery.
